What is Reglan® and Metoclopramide?
Reglan® and metoclopramide are drugs designed to treat gastrointestinal disorders, including gastroesophageal reflux disease and diabetic gastroparesis. The drugs work by increasing the movement of the stomach muscles, which speeds up the rate at which the stomach empties into the intestines. The drug Reglan® is often prescribed in its generic version - metoclopramide.
The Problem with Reglan® - It Can Cause Tardive Dyskinesia
In addition to its intended purpose of treating gastrointestinal disorders, the FDA says that Reglan® and metoclopramide may also cause tardive dyskinesia, a condition which can cause repeated, involuntary movements of the extremities. Other symptoms of tardive dyskinesia include involuntary grimacing, lip movements, blinking, rapid eye movement, sticking out the tongue and problems moving the fingers.
These side effects of Reglan® and metoclopramide are especially serious because tardive dyskinesia has no known treatment and the symptoms of the disease are rarely reversible.
Reglan® (metoclopramide) is prescribed for short-term treatment of gastrointestinal disorders, and is only supposed to be used for under 12 weeks. However, it is often taken for extended periods of time and some estimates suggest that over 30% of all prescriptions are for more than three monhs.
Lack of a Warning
The risk of tardive dyskinesia was not adequately disclosed on the warning label and the manufacturers were aware of the risks associated with long term use of their medications, yet they took no actions to discourage it. In February 2009, the FDA stepped in and required that the manufacturers of all metoclopramide-containing drugs add a "black box" warning about the risk of Reglan® tardive dyskinesia problems . In addition, the manufacturers were required to develop a mitigation and risk evaluation strategy to help ensure that patients are made aware of the potential side effects of Reglan®.
Other Causes of Tardive Dyskinesia
Other drugs, in addition to Reglan®, may cause tardive dyskinesia. It is important for a physician to be consulted to determine if the condition is truly tardive dyskinesia, and to obtain an opinion on the cause of the condition.Tardive Dyskinesia symptoms can range from mild to severe, based on the frequency and intensity of the movements. Other neuroleptic drugs associated with Tardive Dyskinesia include:
- Abilify® (aripiprazole)
- Clozaril® (clozapine)
- Etrafon® (perphenazine, amitriptyline)
- Geodon® (ziprasidone)

- Stelazine® (trifluoperazine)
